3. Print & Mail Your Registration Form. Download and print the Spring 2012 Registration Form below and mail it or drop it off at the Highlands Community Ministries office at 1140 Cherokee Rd. 40204. Forms must be received no later than Saturday, February 11 to ensure placement on a team.
